CARE/Engagement TEAM PROCESS
 
Care Team Purpose:
The Care Team works to help provide support for students and families with basic needs (food, clothing, shelter, etc.), emotional and mental health needs, and physical needs. The Care Team endeavors to engage students and families. We work to remove barriers and connect families to school and community resources. 
 
Care Team Members:
Determined at each school, but will include a minimum of three staff members from the categories below. Administrator
  • School Psychologist
  • Child Development Specialist/Counselor
  • District Nurse
  • Student Advocate (consider) 
  • Teacher
Care Team referrals can be made by:
  • Any staff (via email to the designated Care Team Lead)
  • A parent or guardian (via email, phone call, etc.)
  • A student or family asking for support through a school-wide survey
CareTeam referrals can address the following needs:
  • Basic Needs
    • Food, clothing, shelter, physical medical needs
  • Emotional and Mental Health needs
    • Counseling and referral to community based services
To make a referral for your child or family, contact your studentís building principal
