INSTRUCTIONS TO STUDENTS APPLYING FOR RYLA CAMP
1. Visit the Rotary District 5110 website: http://district5110.org/committees/RYLA
in order to learn more about Camp RYLA and to view videos from previous camps. View the RYLA poster
for a brief overview and camp dates.
2. Download and complete all four (4) required PDF forms:
3. Registration Form:
After you have completed the form electronically, print it, have your parent/guardian review it and sign and date it, and then sign and date it yourself.Please be sure to sign or initial all
places where required!!!
4. Recommendation from school
: E-mail to or print and give the PDF form to a school official, one who knows you well (teacher, coach, etc.) and ask them to complete the form, sign and date it, and return it to you.
5. Petition form:
After you have completed the form electronically, print it, get signed endorsements and contact information from three (3) teachers, initial
your acceptance of the noted conditions, sign and date the form, and then have your parent/guardian review and sign and date the form.
6. Health form:
After you have completed both pages, have it reviewed and signed by your parent/guardian, and initial
your agreement to the no nicotine policy.
7. Submit all four (4) completed forms
to your high school principal or other school official who is in charge of RYLA recruitment.
